Requesting Additional Quantities of Instructional Materials – District Adopted Titles

School-site principals and their designees shall use the Instructional Materials Request Form below to request district-adopted instructional materials. Only school-site principals and their designees are authorized to complete this form.

Before submitting a request for additional instructional materials, principals or their designees must do the following:

  1. Assess the current inventory to identify any material gaps or shortages across all curricula.
  2. Provide a rationale for requesting additional instructional materials, such as:
    • An increase in student enrollment
    • The opening of a new course or grade level
    • Significant damage due to wear and tear
  3. Be prepared to provide a DSIS screenshot to support your request.

Requests will be processed in the order they are received. Depending on the volume of requests, it may take several days to process and create a shopping cart. Once the shopping cart is approved, it will be shared with the Stores & Mail Distribution (S&MD) warehouse, which will review its inventory to determine availability. If additional materials are needed, they will be requested from FSBD. Please note that instructional materials may take up to 6 weeks or more to arrive at the school site, depending on availability at S&MD and FSBD.

Note: Requests submitted before July or after February will not be processed. Only principals or their designees are permitted to make submissions.
